2012 Healthcare, Dental benefit claims submissions
Deadline reminder: March 31, 2013
In order to be eligible for reimbursement of expenses for healthcare and dental items and services incurred in 2012, all claims must be received by Manulife Financial within 90 days following the end of the 2012 calendar year – or by March 31, 2013.
Claim forms for healthcare and dental expenses are available at work locations, the Human Resources Department, on the TTC intranet site, or by registering online at www.manulife.ca/groupbenefits.
Extended healthcare claim forms along with original receipts must be forwarded to:

Reminder from Human Resources
March is Optional Life Insurance month
March is the time that provides you the opportunity to either commence or increase your personal life insurance up to a maximum of $100,000.
Yellow application cards can be obtained from your supervisor and forwarded to Human Resources, Benefit Services no later than March 31.
