Message from Safety and Environment

As of Sept. 30, 2014, the requirements of the Incident Reporting & Investigation Corporate Program are in effect across the TTC.

The Incident Reporting & Investigation Corporate Program updates and clarifies the minimum requirements at the TTC for reporting, recording, investigating, and taking corrective actions on safety, health and environment (SH&E) incidents resulting in or with the potential to result in injury/illness to workers, contractors, customers, and the public or environmental damage.

Investigating and analyzing our incidents for how and why things went wrong allows us to take action on the cause of the incident to prevent it from happening again, and for reducing risk overall.

The TTC has three levels of investigation for SH&E incidents depending on the actual or potential severity of the incident. The examples used below are regarding occupational injuries, the level of investigation conducted for customer/public injuries may be different:

Level 1 – Involves a short investigation to determine the immediate and root causes of the incident by a frontline supervisor who is trained and designated to investigate. Level 1 investigations for occupational injury incidents will focus on incidents where the amount and type of hazardous energy involved would most likely result in a lost-time injury.

Level 2 – Involves a more detailed investigation by Department Level Safety Staff or other qualified staff member into the circumstances to determine immediate causes and root causes. Level 2 investigations for occupational injury incidents will focus on incidents where the amount and type of hazardous energy involved would most likely result in a permanent partial disability or a lost-time injury requiring extensive time off.

Level 3 – Involves a formal team based investigation for our most serious incidents under the supervision of senior management. Level 3 investigations will address incidents where the amount and type hazardous energy involved would most likely result in a fatality or permanent total disability.

ROLES & RESPONSIBILITIES

Workers – Their responsibilities under this program include:

• Immediately reporting incidents to the appropriate frontline supervisor;
• Completing required incident reports, such as an Occurrence Report or Occupational Injury and Disease Report; and
• Co-operating in the TTC incident investigation process.

Supervisors – Their responsibilities under this program include:

• Managing the initial response to an incident by ensuring proper notification, prompt emergency action, securing of the scene, and recording of the incident; and/or
• Investigating further into the incident by conducting a root cause analysis if trained and designated to investigate as per department procedure.

Managers – Their responsibilities under this program include:

• Supporting incident investigations where required;
• Reviewing Incident Investigation Reports and developing corrective action plans as required; and
• Tracking corrective actions to completion.

Department Level Safety Staff – Their responsibilities under this program include:

• Providing investigative expertise and support for all levels of incident investigation as required; and
• Conducting Level 2 investigations as required.

Safety and Environment Department – Its responsibilities under this program include:

• Provide occupational hygiene, environmental, and fire expertise for incident investigations; and
• Maintain incident data and provide statistical reports.
