2015 Healthcare, Dental benefit claims submission

In order to be eligible for reimbursement of expenses for healthcare and dental items and services incurred in 2014, all claims must be received by Manulife Financial within 90 days following the end of the 2014 calendar year – or by March 31, 2015.

In order to be eligible for reimbursement of expenses for healthcare and dental items and services incurred in 2014, all claims must be received by Manulife Financial within 90 days following the end of the 2014 calendar year – or by March 31, 2015.

Claim forms for healthcare and dental expenses are available at work locations, the Human Resources Department, on the Human Resources intranet site, or by registering online at www.manulife.ca/groupbenefits.

Extended healthcare claim forms along with original receipts must be forwarded to:

Dental claim forms along with original receipts must be forwarded to:
Manulife Financial Group Benefits Dental Claims
