For Submission of all 2015 Healthcare and Dental Benefit Claims

DECEMBER 31, 2015 DEADLINE REMINDER
To be eligible for reimbursement of 2015 healthcare and dental expenses for you and your eligible dependents, all claims should be received by Manulife Financial no later than Dec. 31, 2015. Please allow sufficient time for mail delivery. Manulife Financial will honour a 90-day grace period from Dec. 31, 2015, but any claims received after that period (i.e. March 31, 2016) will be declined for reimbursement.
Healthcare and dental expense claim forms are available from:
- Your work location;
- The Human Resources Department;
- The Human Resources Department intranet site; or
- Manulife’s Plan Member Secure Site at www.manulife.ca/groupbenefits.
For your convenience some of your claims may be submitted electronically to Manulife Financial right from the Plan Member Secure Site. For first time users, select Plan Member, then Login/Register. You will need your plan member contract number and certificate number. Then enter your activation key (this code is available from Manulife).
